Impaired hypoxic ventilatory drive induced by diabetic autonomic neuropathy, a cause of misdiagnosed severe cardiac events: brief report of two cases

چکیده
Abstract Background Sudden cardiac deaths are twice more frequent in diabetic patients with autonomic neuropathy. death etiologies remain unclear and no recommendations made to identify factors associated cardiorespiratory arrest patients. We hypothesized, from two clinical cases, that impaired hypoxic ventilatory drive, induced by neuropathy, is a cause of misdiagnosed severe events. Case presentation describe the cases isolated low blood saturation on pulse oximeter during systematic nurse check-up (77% 85% respectively) contrasting absence any complaint such as dyspnea, polypnea or other respiratory insufficiency signs observed examination. Arterial gas measurements subsequently confirmed oxygen was both were indeed hypoxemic. Patient 1 suffered vascular overload complicated caused hypoxemia light quick recovery after ventilation. Pulmonary edema diagnosed patient 2. The common denominator these 2 described this brief report failure presence hypoxemia. Also, precursory seems be an drive This appears related neuropathy encountered those Conclusions Therefore, we describe, report, involved acute events type assume altered response due non-functional central neurological breathing command could play key role sudden among An important point can easily missed since reported cases. Systematic screening proactive detection for early management (e.g. treatment hypoxemia) should systematically undertaken prevent its dramatic consequences death.

Correction of severe tooth rotation by using two different orthodontic appliances: Report of two cases
BACKGROUND AND AIM: Severe rotation of tooth is one of the most common problems in orthodontics and considered as a developmental phenomenon. These rotations can cause cosmetic problems, gingival recession, and traumatic occlusion. By using removable appliances, severe rotations can be treated. Furthermore, gingival damage, tooth attrition, and transposition of other teeth could be prevented. E...
متن کاملPreservation of the hypoxic drive to breathing in diabetic autonomic neuropathy.
1. Unexplained cardiorespiratory arrests have been reported in patients with diabetic autonomic neuropathy and these could be due to denervation of the carotid chemoreceptors. 2. We have studied the ventilatory response to transient hypoxia (Ve/Peto2) during exercise in 22 male diabetic patients, six with symptomatic and cardiovascular evidence to suggest diabetic autonomic neuropathy (DAN+) an...
متن کاملAnt-induced Alopecia: Report of two cases
Localized scalp hair loss has different causes which alopecia areata, trichotillomania, tinea capitis and early lupus erythematosus are the most important ones. There are several reports of localized alopecia after tick and flea bites and bee stings, but there is only one report of ant-induced alopecia in the literature. We report herein two cases of alopecia induced by ants of genus Phei...
متن کاملVentilatory response to exercise in diabetic subjects with autonomic neuropathy.
We have used diabetic autonomic neuropathy as a model of chronic pulmonary denervation to study the ventilatory response to incremental exercise in 20 diabetic subjects, 10 with (Dan+) and 10 without (Dan-) autonomic dysfunction, and in 10 normal control subjects. Although both Dan+ and Dan- subjects achieved lower O2 consumption and CO2 production (VCO2) than control subjects at peak of exerci...

one of the important complications of diabetes mellitus that physicians pay less attention to is diabetic autonomic neuropathy(DNA) ,especially cardiac autonomic neuropathy (CAN).this has a poor prognosis and is accompanied by sudden death risk, recently several investigations that were carried out for CAN detection ,suggested that QT-interval prolongation in these patients is an important diagn...
